There are five items that must be returned to the Agency Relations Manager in order to process your request to become a Partner Agency of the Foodbank of Southeastern Virginia and the Eastern Shore. Click on each link below to print out these required application components:
1. The application form
2. The Criteria for Membership
3. The Acknowledgement and Release
4. The Annual Membership Fee (AMF) form
5. You must also submit a copy of your 501(c)3 or 14 point letter.
Please be sure to submit all five of the above items as your application will not be processed without them.
As you can imagine, November and December are our highest volume times of the year. As a result, we will not process new applications for partnership in November or December.
