Application Components
There are five items that must be returned to the Agency Relations Manager in order to process your request to become a partner agency of the Foodbank of Southeastern Virginia. Click on each link below to print out these required application components:
1. The application form
2. The Criteria for Membership
3. The Acknowledgement and Release
4. The Annual Membership Fee (AMF) form
5. You must also submit a copy of your 501(c)3 or 14 point letter.
Please be sure to submit all five of the above items as your application will not be processed without them.
